- Graduate applicants will also need two letters of recommendation. Please see application form for more information.
- Once printed, complete the documents and mail it to the address indicated.
- You may also fax the documents to the Tulsa location at (918) 610-1111 or the Shawnee location at (405) 878-5198. Please be sure to fax the application to the attention of your Enrollment Representative.
